I must be dumb and blind, but still I cannot find it anywhere. Am sure I didn't collect it yet, must be lying somewhere around.. just cannot find it

I have been browsing forum for a while, seems there should be at least one Toorum's note (on GameBanshee they say 3), but am still not able to find even one of them. Map with positions would help a lot.
