In hard mode, every monster is deadly. No one can be tanked. You need to strafe hit and run :( Even a low monster can kill your front line in case of RNG. Ok there is no bug. The rage ability is not working only when life is below 20%, it begins when life goes below 20% and stay for a duration (STR bonus is lowered during time).

Ok case 2 seems ok (I thought it was 25% instead of 20%).
