On lvl 5:
There are 2 Toorums "notes", 1 "Scroll" from him and a Note, the scroll is found in the middle pit i think.
Lvl 6:
+1 Secret
Edit: At the demon statue (the one you cast a fireball), turn north and walk to a dead end. Scan the wall there for a runic symbol - press it and a hidden door will be revealed. Entering the door, which leads to the Fighter's Challenge, counts as a secret.
So lvl 6: 11 Secrets

Toorum's notes in level 5: Do you mean 1 of them is found when you jump into a pit? In that case, the note would be assigned to level 6

I love lists like this. They don't give anything away spoilers-wise but let you know just what, if anything you have missed.

No problem, i missed a treasure (6/7) and a bundle of secrets (63/71) on my first playthrough, so i'd like this list finished so it can help many people like me, wishing to get everything

There is. It is inside a helmet in the later half of the level.
I wasn't aware of there being a treasure on floor 2. I have done my best to scour ever inch of the first 4 floors(just restarted this morning with even better results), and I haven't seen it. Any tips on where?
Can't wait to see some maps with these items listed on them.
EDIT: Sorry, it is not down any chasm. It is in the back of an alcove that opens up after a ton of spiders are killed. There should also be some tar beads with it too.

Cool. It appears I was wrong with the how to get that skull. Or how the room opens, rather. There's a switch I found, and I'll have to load my game back up in a bit locate it again.
Maybe I should make some maps as well...
