STUCK (BUG?) at Level 4 scrolls room (door wont open)

Hi, I'm new to this forum. I really love this game! So classic! Cancelling my Diablo 3 pre-order on local store, I bought this game instead at May 14 via GOG. lol.

Anyway, I managed to get into Level 4. I obtained all the scrolls but when I put them in the slot, nothing happened. No doors opened. I checked the gamefaqs walkthrough and it says that we supposed to open some kind of door on the northern and southern wall in the portal room. Also, there supposed to be a 'scavanger' monster that we must figt.
I think I put the scrolls correctly. Sleep -> Dream, Crimes -> forgiven, Cave -> echo, imprison- > call--something like that.

Is it some kind of bug or am I doing it wrong? Should I do enter the portal in order?

Btw, after I exit the Trail of Thoughts portal, there's a hydra-like monster popped out from the sewer door just. (is this the scavanger?)

Yeah, because they just suddenly popped out! Fortunately, they don't move around and pretty easy to defeat.
